Cortisol face is real-ish. Here's what actually de-puffs.
'Cortisol face' isn't a medical diagnosis. A truly rounded 'moon face' comes from very high cortisol over a long time, like Cushing's syndrome or long-term steroid use, not a stressful week. Everyday puffiness usually comes from short sleep, salt, alcohol and lying flat. Cold, gentle pressure and better sleep de-puff best.

Where "cortisol face" came from
Cortisol is your main stress hormone. It rises and falls all day, highest in the morning. The trend took a real medical fact, that very high cortisol over a long time can change where your body stores fat, including in the cheeks, and stretched it to cover anyone who's stressed and puffy.
Here's what doctors told TODAY:
- Dr. Shari Lipner, a dermatologist at Weill Cornell Medicine: "I don't think we should blame cortisol." And: "Fluctuations in your cortisol levels with stress does not cause a moon face."
- Dr. Priya Jaisinghani, an endocrinologist at NYU Langone, explained that a moon face can come from conditions affecting the pituitary or adrenal glands or from medications like prednisone, and that "cortisol face" isn't a diagnosis.

So the "real-ish" part: cortisol can change your face. Just not because your inbox was bad this week.
What's actually making you puffy
The everyday culprits, per those same experts: poor sleep, alcohol, an imbalanced diet, and extra salt or sugar. Add gravity. When you lie flat for eight hours, fluid redistributes into your face, especially under the eyes where skin is thin. Stand up and it slowly drains. That's why you look different at 7am and 10am.
The fix has two layers: the morning rescue, and the stuff that makes tomorrow morning easier.
The 10-minute morning de-puff
1. Cold, first
Cold narrows blood vessels and just feels good on a puffy face. Keep your eye patches in the fridge. A metal spoon from the freezer works too, and costs nothing.
The Grace & Stella masks are $26, which makes them a random-Tuesday thing, not a special-occasion thing. That's the point.
If your whole face (not just eyes) is the problem:
2. Move the fluid
Gentle, directional pressure helps push fluid toward the lymph nodes in your neck. Light pressure, outward from the center of your face, then down the sides of your neck. Five minutes.
The best facial-specific study we found, a small 2025 trial, showed modest contour changes after eight weeks of regular sessions, which Dermatology Times attributes to a fluid shift, not sculpting (Dermatology Times). For a puffy morning, a fluid shift is exactly what you want.
The $10 terracotta brush is the one to toss in your bag.
3. A cooling de-puff step
Caffeine is a classic ingredient in de-puffing products, and a cool gel texture does a lot of the work on its own. Fridge it for extra credit.
At $12.99, the Byoma gel is the budget pick.
4. Conceal, don't cake
Puffy under-eyes plus thick concealer equals creasing by 10am. Go thin, tap it in, and set only where it creases.
Setting up tomorrow morning
The trick is making tomorrow's 7am easier:
- Sleep a little propped up. An extra pillow helps fluid not pool in your face.
- Watch the late salty dinner and the third drink. Not forever. Just notice the pattern.
- Protect your sleep. CDC's basics: the same bedtime and wake time every day, a quiet, cool room, devices off at least 30 minutes before bed, and no large meals or alcohol right before bed (CDC). Our Night Shift ritual turns that into three steps.
- An overnight mask if you like the ritual. It won't de-puff you while you sleep, but a hydrated morning face looks less tired.
What doesn't help (much)
A few things that show up in "cortisol face" videos, sorted honestly:
- Cortisol-lowering supplements sold for your face. We couldn't find good evidence that any supplement changes facial puffiness. If you suspect a hormone issue, that's bloodwork with a doctor, not a powder.
- Cutting all salt forever. You don't need to. Notice whether the puffy mornings follow the salty, late dinners, then adjust the timing rather than the whole menu.
- Aggressive face massage. More pressure doesn't move more fluid. It just raises the odds of bruising or broken capillaries, especially if you have rosacea or thin skin. Light strokes, always.
- Ice baths for your face every day. Cold helps, but a chilled patch or spoon does the job. Your face doesn't need a polar plunge.
- Panic-buying a new ritual. Morning puffiness is normal. You don't need 9 products to look like you slept. You need 10 minutes and, ideally, actual sleep.
And the stress part
The experts TODAY spoke with were clear that lowering stress won't change your face shape. They also said it's still good for your skin and your overall health. Both can be true. You don't need to fix your face to deserve a calmer evening.
If something does feel off, trust that. See a doctor if:
- Swelling comes with trouble breathing or swollen lips or tongue (that's an emergency).
- Your face is gradually getting rounder alongside other changes like weight gain around your middle, easy bruising, purple stretch marks or high blood pressure.
- Swelling is one-sided, painful or getting worse over days.
Otherwise: fridge, stone, thin concealer, bed on time. You look fine. You look like someone who woke up.

FAQ
Asked and answered
Is cortisol face a real thing?
Not as TikTok describes it. Doctors interviewed by TODAY said daily stress fluctuations don't cause a moon face. That requires very high cortisol for a long time, usually from a medical condition or medication.
Why is my face puffy in the morning?
Fluid collects in your face while you lie flat overnight. Salty dinners, alcohol and short sleep make it worse. It usually settles within an hour or two of being upright.
How do I get rid of a puffy face fast?
Cold (chilled eye patches, a cold spoon or a cooling mask), then gentle outward strokes with a gua sha or roller, then a concealer that doesn't crease.
Can lowering stress change my face shape?
Not your face shape, according to the experts TODAY spoke with. But less stress and better sleep are good for your skin and the rest of you.
When should I worry about facial swelling?
Swelling with trouble breathing or swollen lips is an emergency. Gradual rounding of the face alongside weight gain, easy bruising, stretch marks or high blood pressure is worth a doctor's visit.
